Requirements
- Successfully completed a Home Health Aide training program and competency evaluation as required by Medicare Conditions of Participation.
- Demonstrates a compassionate and caring attitude toward the sick and elderly.
- Ability to follow directions, read, and write effectively to carry out assigned tasks.

Responsibilities
- Provide personal care, bathing, and hands-on care as outlined in the patient’s plan of care.
- Complete visit records promptly and accurately according to Agency policy.
- Report any changes in the patient’s condition to the supervising RN.
- Perform essential household tasks to support health care in the home as assigned.
- Assist patients with ambulation and exercises as directed by the care plan.
