About The Position

Hospice of the East Bay (HEB) is a community-based nonprofit that provides hospice and palliative care services to support and comfort individuals and their loved ones through the final stages of life. This internal posting is for a Home Health Aide position that will support patients in all of HEB's covered service areas. The position is responsible for carrying out assigned duties to enhance patient comfort, maintain personal hygiene, and ensure a safe and healthy environment for the patient. While reporting to a primary manager, the Home Health Aide may work across different programs or departments to meet patient needs.

Requirements

  • Must be able to work collaboratively as a team with strong communication skills and be flexible with work assignments and professional when dealing with clients and staff.
  • Must adhere to department goals, objectives and performance standards.
  • Able to establish and maintain cooperative relationships with internal and external customers.
  • Able to work well under pressure and handle multiple, changing and competing deadlines.
  • Must be able to understand and carry out directions and instructions and maintain patient documents.
  • Must be physically able to perform duties as assigned.
  • Must have reliable transportation.
  • Must have an HHA License to be considered for the position.
  • Must provide 6 shifts per month minimum, including 2 weekend days.

Responsibilities

  • Performing personal care needs and attending to patient requests after receiving an assignment, communicating with the Case Manager if unable to perform a task.
  • Providing positive communication techniques to patients and families, maintaining confidentiality and ethical care.
  • Meeting patient safety needs and using equipment safely and properly.
  • Following the plan of care as established by the RN.
  • Demonstrating clinical care within the Home Health Aide scope of practice.
  • Assessing and reporting on patient condition and significant changes to the RN, and being aware of caregiver or other individuals living with the patient and interpersonal issues.
  • Recognizing and reporting emergencies.
  • Keeping the patient’s living area clean and orderly as assigned.
  • Adhering to the organization’s documentation, care procedures, and standards of personal and professional conduct.
  • Participating in quality assessment performance improvement teams and activities.
  • Participating in IDT meetings.
  • Showing respect for the patient and family/caregiver environment and patient’s personal needs.
  • Maintaining inventory of disposable supplies.
  • Maintaining cleanliness and infection control standards.
  • Performing other duties as assigned.
