The incumbent assists patients and their families with ADLs and IADLs in their place of residence and functions as a member of the Hospice Interdisciplinary Team. As our Home Care Aide, you will provide essential personalized support and companionship to clients in their homes, enhancing their overall quality of life and fostering independence according to individualized care plans. Every day you will assist clients with activities of daily living (ADLs) such as personal care, meal preparation, medication reminders, light housekeeping, and transportation to appointments. You will meticulously document services provided and proactively communicate any changes in the client's condition to the appropriate supervisor. To be successful in this role, you will possess a compassionate and patient demeanor, excellent communication and interpersonal skills, and a strong commitment to client dignity and well-being. Your ability to adapt to varying client needs, maintain a safe environment, and adhere to care plans with attention to detail is essential. Demonstrates competency in patient care during field observation by the Clinical Supervisor/designee. Promotes/encourages patient/family independence as evidenced by direct observation. Completes and submits documentation following established guidelines, as measured by a designated evaluation tool. Demonstrates positive attitude and flexibility in meeting patient's needs as evidenced by reports from the PICs regarding willingness to accept patients and schedule changes. Care follows the Home Health Aide Plan of Care on each visit as evidenced by field observation and review of visit notes. The yearly average AVD is equal to the discipline-specific standard.
