As a Home Health Aide on our Hospice team, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. This position will support various counties in the Southern region which include Summit, Medina, Portage, Wayne and various others. Requirements

• High School Diploma or GED
• State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry
• One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting
• Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation period

Nice To Haves

• Hospice and Palliative Care Nurse Assistant (CHPNA) Certification
• Home Care experience

Responsibilities

• Perform basic patient care activities.
• Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
• Assist patients with tending to personal care, activities of daily living and transfers/transport.
• Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
• Assist with keeping the patient's environment clean and orderly.
• Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
• Escalate change in patient condition to patient’s physician as appropriate.
