About The Position

Sharp Hospice Homes is seeking a Full Time Home Health Aide for the Night Shift. This position is covered by a Collective Bargaining Agreement (CBA) with SEIU-UHW. As part of the terms of employment, employees in this role are required to join the union within 31 days of hire and remain a member for the duration of the collective bargaining agreement. This position will be posted for 7 days internally from 05/07/26 to 05/14/26 and available only to existing members of the ratified SEIU Bargaining Unit with a contract expiration date of 9/30/2027. The Home Health Aide will collaborate with interdisciplinary teams in the delivery of quality patient care under the direction of a registered nurse, assist in performing personal care needs and compassionate end-of-life care, and contribute to overall unit functions.

Requirements

  • California Home Health Aide (HHA) - CA Department of Public Health -REQUIRED
  • California Certified Nurse Assistant (CNA) - CA Department of Public Health -REQUIRED
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
  • Driver's License - CA Department of Motor Vehicles -REQUIRED
  • Utilizes reliable transportation and possesses adequate personal insurance coverage.
  • Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.
  • Knowledge of medical terminology and techniques.
  • Ability to execute Home Health Aide skills safely.
  • Ability to effectively communicate and function in a team environment to enhance patient care.

Responsibilities

  • Collaborate with the interdisciplinary teams in the delivery of quality patient care under the direction of a registered nurse.
  • Assist in performing personal care needs and compassionate end-of-life care.
  • Contribute to overall unit functions.
  • Observe patient condition and report changes to the RN.
  • Report pertinent patient related information to RN.
  • Observe changes in patient's condition such as declining ability to participate in ADL's, changes in skin condition, level of consciousness, pain management, etc.
  • Promptly report appropriate changes in patient's status to RN.
  • Effectively communicate and participate in the coordination of the plan of care to meet established goals.
  • Perform routine equipment maintenance according to departmental policy.
  • Maintain all certifications required of the job.
  • Demonstrate and maintain expertise through attendance and participation in department in-services and meetings.
  • Understand and demonstrate effective communication to patients/families in alignment with hospice philosophy and patients plan of care.
  • Provide appropriate discipline specific information on end of life issues to patient/families based on their needs.
  • Listen actively and provide a meaningful presence to facilitate the identification and discussion of patient goals and preferences as it relates to personal care.
  • Promote rapport to foster patient relationship to support patient throughout their hospice care.
  • Provide comprehensive personal care to patients under the supervision of the RN.
  • Appropriately interact with patients/families.
  • Adhere to plan of care.
  • Demonstrate functional knowledge and skills at a competency level.
  • Perform comfort and safety measures to ensure that patient's needs are met.
  • Perform simple nursing procedures under the supervision of the RN (i.e. vital signs).
  • Promote and develop relationships based on trust, respect, and rapport.
  • Maintain privacy and confidentiality at all times.
  • Demonstrate and maintain expertise through attendance and participation in department in-services and meetings.
  • Maintain individual in-service/performance records.
  • Demonstrate process of self-discovery including knowledge of strengths and weaknesses and view deficiencies as opportunities for growth.
  • Maintain a safe working environment for patients, families and other members of the team.
  • Comply with universal precautions and Department infection control policies.
  • Consistently utilize proper body mechanics and available support equipment to ensure patient's safety and avoid personal injury.
  • Identify and communicate patients who are at risk for injury to self and others.
  • Understand and demonstrate the team member role and responsibilities.
  • Demonstrate effective and positive working relationships.
  • Convey a willingness to help and promote cooperative team efforts.
  • Demonstrate an ability to perform and effectively communicate with other team members.
  • Contribute ideas/suggestions to improve quality comfort care needs of the patient.
  • Elicit assistance from team members to problem-solve and respect the opinions from others.
  • Participate in shift reports, effectively communicating discipline specific information.
  • Appropriately refer to appropriate discipline for matters outside their scope of practice.
