Requirements
- Comfort with Home Visits: Be comfortable and confident visiting patients in their homes across the Park Rapids area.
- Reliable Transportation: Have dependable transportation to provide timely and reliable care to our valued clients.
Responsibilities
- Empower Independence: Visit clients in their homes where you'll provide essential support that empowers them to maintain their independence. Home is more than a place—it's where you make a lasting impact.
- Support Daily Living Activities: Assist clients with everyday activities, from mobility support to personal hygiene, ensuring they live in a clean and safe environment.
- Build Relationships: Forge genuine connections with your clients, becoming a trusted part of their lives. Collaborate closely with our Nurse Case Managers to promptly address and report any health concerns.
