About The Position

We're hiring dedicated, dependable Home Health Aides (HHA). This role is ideal for caregivers who want steady, meaningful engagement with clients and are ready to take on consistent daily shifts providing assistance with ADLs and dementia. Our 24/7 support model ensures you're never alone in the field and always backed by a responsive, reliable office team. Candidates should be confident and experienced in full-assist care and mobility support. The Schedule: 2:00 PM - 5:00 PM, Any Day on Mondays and Thursdays. Requirements

  • High school diploma or GED
  • Minimum one (1) year of documented home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred
  • Successful completion of a nursing assistant or certified home health aide program
  • Certified CPR and negative TB skin test or chest x-ray
  • Must have a valid driver's license and auto insurance (proof of insurance is required)
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client.
  • Must be able to lift up to 50 pounds
  • Self-directing with the ability to work with little direct supervision
  • Ability to adhere to HIPAA and ensure confidentiality and security of the client's medical information
  • Ability to read, write, speak, and understand English and communicate effectively
  • Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B

Responsibilities

  • Provide assistance with ADLs, dementia, including bathing, dressing, grooming, toileting, and personal hygiene
  • Support mobility and transfers, ensuring safe ambulation and assisting as needed
  • Assist with meal preparation and provide medication reminders as directed
  • Offer companionship and emotional support to ensure client comfort, dignity, and well-being
  • Monitor and report any changes in the client's condition to supervisors or case managers
  • Follow the established care plan and maintain clear communication with the care team
  • Provide client-facing care as assigned
  • Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping
  • Provide companionship, personal and hygiene care as directed in the client's personalized plan of care, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities, and assisting with measurements, if necessary
  • Assist clients with active and passive mobility work defined by the plan of care, and document observed changes to the Director of Nursing
  • Adhere to the policies, procedures, and protocols of the agency of employment
  • Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information
