We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Requirements

Holds a high school diploma or equivalent.
Minimum of 6 months of experience as a Home Health Aide or in a related caregiving role.
Valid New Jersey Certified Home Health Aide (CHHA) certification is required.
Strong compassion, empathy, and a genuine desire to help others.
Excellent communication and interpersonal skills to build rapport with patients and families.
Patience, reliability, and a positive attitude.
Ability to follow instructions and adhere to safety protocols.
Graduated from an accredited Home Health Aide program
High School Diploma or GED
One year prior professional experience
Driver’s license required
CPR certification required

Nice To Haves

Prior Home Health or Hospice experience is a plus.

Responsibilities

Assist patients with personal care needs such as bathing, dressing, grooming, and toileting.
Help patients with ambulation and transfers, ensuring their safety and mobility.
Prepare and serve meals, considering dietary restrictions.
Light housekeeping tasks to maintain a clean and safe home environment.
Monitor patients' vital signs and report any changes to healthcare professionals.
Provide companionship and emotional support, fostering a positive and caring environment.
Document patient care accurately and completely.
Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
Give medications and immunizations
Engage patients in exercises or other activities
