We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Requirements

  • Holds a high school diploma or equivalent.
  • Minimum of 6 months of experience as a Home Health Aide or in a related caregiving role.
  • Valid New Jersey Certified Home Health Aide (CHHA) certification is required.
  • Strong compassion, empathy, and a genuine desire to help others.
  • Excellent communication and interpersonal skills to build rapport with patients and families.
  • Patience, reliability, and a positive attitude.
  • Ability to follow instructions and adhere to safety protocols.
  • Graduated from an accredited Home Health Aide program
  • High School Diploma or GED
  • One year prior professional experience
  • Driver’s license required
  • CPR certification required

Nice To Haves

  • Prior Home Health or Hospice experience is a plus.

Responsibilities

  • Assist patients with personal care needs such as bathing, dressing, grooming, and toileting.
  • Help patients with ambulation and transfers, ensuring their safety and mobility.
  • Prepare and serve meals, considering dietary restrictions.
  • Light housekeeping tasks to maintain a clean and safe home environment.
  • Monitor patients' vital signs and report any changes to healthcare professionals.
  • Provide companionship and emotional support, fostering a positive and caring environment.
  • Document patient care accurately and completely.
  • Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
  • Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
  • Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
  • Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
  • Give medications and immunizations
  • Engage patients in exercises or other activities
