About The Position

Concordia Visiting Nurses is seeking a compassionate, skilled, and dependable Home Health Aide (HHA) to join their growing team. This role involves providing personalized support to individuals in their homes, offering high-quality, person-centered care that respects each client's independence and dignity. The agency is Medicare-certified, state licensed, and Medicaid approved, serving nine Western Pennsylvania counties with skilled nursing, palliative care, and therapy services for over a decade. Their mission is to deliver quality home health services with respect, dignity, and compassion, while striving to hire and retain excellent people.

Requirements

Strong interpersonal and communication skills to build rapport and trust with clients and families.
Empathy, compassion, and a patient demeanor.
Reliability, adaptability, and the ability to handle unexpected situations with composure.
Basic medical knowledge, including vital signs monitoring, medication administration, and infection control.
Physical stamina to assist with mobility and transfers.
Relevant certifications, such as CPR, First Aid, or HHA/CNA certification (or a willingness to obtain them).
A high school diploma or equivalent.
A clean driving record and reliable transportation (for roles requiring travel).

Responsibilities

Assist clients with daily activities, including bathing, dressing, grooming, and personal hygiene.
Provide companionship, emotional support, and engage in activities to promote well-being.
Assist with mobility and transfers, including the safe use of assistive devices like wheelchairs and walkers.
Monitor and record vital signs, reporting any significant changes to the healthcare team.
