As a Home Health Aide, you will play a vital role in helping patients maintain dignity, independence, and comfort in their own homes—while being supported by a collaborative and experienced clinical team.
Requirements
- Completion of a Home Health Aide training program meeting Medicare Conditions of Participation
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel to patient homes throughout the day
- Strong communication and interpersonal skills
Nice To Haves
- Previous home health experience
- CNA certification
- Experience working with diverse patient populations
Responsibilities
- Assist with personal care, including bathing, grooming, and hygiene
- Support patients with daily living activities while promoting independence and dignity
- Observe and report changes in patient condition to the supervising RN
- Follow the established plan of care to ensure safe, effective service delivery
- Accurately document each patient visit in accordance with agency policies
- Maintain compliance with all regulatory and company standards
