About The Position

Requirements

• Active CHHA certification
• High School Diploma or GED
• Minimum 1 year of direct patient care experience (home health preferred)
• Reliable transportation and valid driver’s license
• Self-motivated with a passion for helping others

Responsibilities

• Provide high-quality, compassionate in-home care
• Assist patients with daily activities and personal care
• Maintain accurate documentation of care provided
• Communicate effectively with the care team and families
• Travel daily to patient homes (reliable transportation required)
