About The Position

Home Care Network (HCN) is seeking 2-3 highly-skilled, compassionate, and professional Home Health Aides (STNA or CNA highly desired, but experienced HHA, CHHA, DSP, or PCA will also be considered) to join their home health agency. The role involves providing hands-on personal care and support to clients in Carroll County, OH, including Carrollton, OH, and surrounding areas. A successful Home Health Aide at HCN provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients and perform services as necessary to maintain personal comfort and safety in their home. HCN is a nurse-owned and locally led home health agency prioritizing patient care and well-being. They aim to create an environment where nurses and caregivers thrive, empowering communities' most vulnerable members with independence.

Requirements

Minimum of 1 year paid in-home care or caregiving experience (STNA/CNA certification in lieu of experience will be considered).
Successful completion of all agency-provided HHA/PCA orientation, skills tests, and competency evaluations.
Willingness and timely compliance to maintain all ongoing Training & Certification Requirements.
Active CPR + FA certification and documentation of both, or willingness to complete through HCN within 2 weeks / prior to hire.
Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with the agency.
Ability to perform all physical demands of home care on a regular basis, including frequent standing, walking, stopping, ending; infrequent squatting/kneeling, balancing; pushing and handling weight/force of 1-20 lbs. frequently, 20-30 lbs. occasionally, and 30-50 lbs. infrequently; lifting and carrying 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently.
Exceptional on-time and attendance record, and consistent availability aligned to chosen schedule type (PT/FT/Hybrid).
Character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
Good interpersonal skills and compassion for helping others live their best lives.
Consent and ability to pass a State & Federal background check (including fingerprints).
Must be eligible to work in home and community-based settings (HCBS).
Must be at least eighteen (18) years of age.
Must possess an active Driver's License / State ID.
Must have US Work Authorization.
Must have reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients.

Nice To Haves

3-5 years of paid experience in a home health or caregiving setting.
Paid experience with a Home Health Agency.
Experience with G-tubes / feeding support.
Experience with Hoyer Lifts.
Experience with Seizure protocols.
Experience with Behavioral health or crisis response.
High School diploma or GED.
Formal certification or training program: STNA (or 2026 graduation date) highly preferred; CNA or CHHA still encouraged to apply.
Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
EMR/EHR experience and proficiency (ContinuLink highly desirable).
Familiarity with Medicaid waiver services.
Medication Administration Certification is highly preferred.
Willingness to serve clients throughout Carroll County, OH (~30-40 min radius).
45–60 min radius travel is preferred but not required.
Caregivers committed to long-term (2+ year) employment are preferred.

Responsibilities

Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
Measure and record vital signs and other specified metrics as instructed.
Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
Perform household tasks essential to healthcare and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
Assist patients in taking medications under the direction and supervision of the Registered Nurse (opening containers or pill caddies and handing them to the patient).
Identify and report any abnormal findings or changes in the patient's condition.
Help with therapy exercises under the supervision of therapists (e.g., range of motion exercises, ambulation assistance).
Provide Personal Care & Activities of Daily Living (ADLs) including safe transfers, mobility, and positioning support.
Support safe use of assistive devices (walkers, wheelchairs, lifts).
Provide Nutrition & Meal Support.
Perform Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment.
Provide Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation.
Perform Basic Health and Safety Monitoring.
Implement Care Plans.
Provide Dementia & Behavioral Support (if applicable).
Perform pro-active observation and excellent electronic documentation.
Uphold Client Rights, Dignity & Confidentiality.
Engage in Total Care Team Collaboration.
Participate in Ongoing Training & Certification Requirements.
