About The Position

Home Care Network (HCN) is seeking compassionate, dependable, and experienced caregivers to join our team in a PRN (as-needed) capacity. This opportunity is ideal for healthcare professionals seeking supplemental income, schedule flexibility, or a pathway to future part-time or full-time opportunities. This is a PRN / As-Needed position only. Hours vary significantly from week to week, with no minimum number of hours guaranteed. Shift opportunities are based entirely on client needs, business demand, and caregiver availability. Willingness to travel to and from client residences is required, with a desired travel radius of up to 30-60 minutes from home. Most available assignments occur during weekdays, with potential for weekend opportunities. After approximately 30 days of employment, PRN caregivers may be considered for additional client assignments to increase hours into a part-time or full-time schedule, though increased hours are not guaranteed. A successful Home Healthcare Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region, performing services for clients as necessary to maintain personal comfort and safety in their home. This includes delivering direct patient care, measuring vital signs, assisting with personal care tasks, performing household tasks, assisting with medication reminders, identifying and reporting changes in patient condition, and assisting with therapy exercises under supervision.

Requirements

  • Possess a strong track record of attendance, dependability, and employment retention with previous employers.
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Ability to perform all main physical tasks on a regular basis, and willingness to do so safely.
  • Good interpersonal skills and compassion for helping others live their best lives.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Professional character traits: reliable, honest beyond reproach, punctual, articulate, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, self-driven, problem-solver, compliant, and confidential.
  • Be at least eighteen (18) years of age.
  • Valid driver's license/ID and reliable transportation to/from client care locations.
  • Consent and ability to pass a State and/or Federal Background Check.
  • 1-3 years of verifiable experience in home care or caregiving setting (based on credentials).
  • Successful completion of HCN's written skills test and/or competency evaluation.
  • Meet/Completion of all training and/or certifications required by the state in which the home health aide services are being provided.
  • CPR and First Aide Certifications (active/current certs preferred but may also be obtained prior to hire date through HCN or accredited third party).

Nice To Haves

  • Active Ohio STNA or CNA certification; and Minimum one (1) year of paid caregiving experience.
  • Active Ohio DODD DSP Certification; and Minimum one (1) year of paid experience supporting individuals with disabilities; and Willingness to provide care for both DODD and non-DODD clients.
  • Minimum three (3) years of paid home care, caregiving, HHA, or PCA experience without certification.
  • Additional experience with medication reminders, transfers, gait belts, Hoyer lifts, dementia care, specialized disease-state support, and related caregiving skills is highly desirable.
  • 3-5 years of paid experience in a home health or caregiving setting.
  • Paid experience with a Home Health Agency.
  • Formal certification or training program: STNA (or 2026 graduation date) highly preferred; CNA or CHHA still encouraged to apply.
  • Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
  • EMR/EHR experience and proficiency (ContinuLink highly desirable).

Responsibilities

  • Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient but does not include administration of the medications.
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
