About The Position

As a Home Health Aide, a member of the home care and Hospice teams, you will aide as a paraprofessional member of the home care and hospice teams who work under the supervision of a registered nurse or therapist and perform various personal care services as necessary to meet the patient’s needs. Use your critical thinking skills in observing patients, reporting these observations, and documenting observations and care performed.

Requirements

• Valid Driver's License and automobile that is insured in accordance with organizational requirements and is in good order.
• Certified Nursing Assistant Certification (CNA) required for employees hired after year 2010.
• BLS Certification by American Heart Association required in timeframe specified in CPR (BLS)/ALS/PALS Policy

Nice To Haves

• One (1) year experience in home health or hospice preferred.
• High School Diploma preferred.
• Certified Hospice and Palliative Nursing Assistant (CHPNA) preferred.
• BLS Certification by American Heart Association preferred upon hire

Responsibilities

• Perform various personal care services as necessary to meet the patient’s needs.
• Observe patients, reporting these observations, and documenting observations and care performed.
