In this role, you provide essential personal care services and monitor patients’ well‑being to support their comfort, safety, and independence at home.

Requirements

  • Active California Certified Home Health Aide (CHHA) license (required)
  • Minimum 1 year of patient care experience , ideally in a Home Health or Home Care setting
  • Valid driver's license and insured, reliable vehicle
  • Strong communication skills and a passion for compassionate, patient‑centered care

Responsibilities

  • Provide hands‑on personal care, including:
  • Bathing, grooming, oral hygiene, shampoos
  • Skin and nail care
  • Dressing/undressing assistance
  • Toileting and incontinence care
  • Changing bed linens
  • Maintaining a clean and safe patient environment
  • Assist with ambulation, mobility, and prescribed exercises
  • Plan, prepare, and serve nutritious meals
  • Assist with feeding , if needed
  • Take and record vital signs (temperature, pulse, respiration, blood pressure) in accordance with competency and plan of care
  • Support patients with self‑administration of medications
  • Identify and report changes in condition to the supervising nurse
  • Use home care equipment safely and correctly
