As a fast-growing, leading provider, we proudly serve patients across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah. We are looking for a compassionate and skilled home health aide (HHA) or certified nursing assistant (CNA) in Hunters Point, AZ to administer patient care in accordance with a physician established care plan. This role is a 1099 contract opportunity, providing care at the patient's residence. A home health aide or certified nursing assistant is needed for 28 hours a week. The pay range is $17 to $18 per hour, depending on experience.

Requirements

  • Home health aide (HHA) or certified nursing assistant (CNA) certification
  • Compassionate and skilled in patient care

  • Ability to provide care at the patient's residence
  • Availability for 28 hours a week

Responsibilities

  • Provide compassionate, one-on-one care to patients in the comfort and familiarity of their own homes.
  • Assist patients with activities of daily living (ADLs), including personal care and hygiene.
  • Perform light housekeeping duties to help maintain a safe, clean, and comfortable home environment.
  • Prepare nutritious meals according to the patient's dietary needs and preferences.
  • Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration, in accordance with the patient's plan of care.
  • Provide medication reminders to help ensure medications are taken as prescribed (without administering medications unless authorized).
  • Maintain accurate and timely documentation of care provided, patient observations, and any changes in condition.
  • Communicate effectively with family members, caregivers, and other authorized individuals regarding the patient's health status, care needs, and any concerns.
