About The Position

Community Home Health Care is seeking compassionate and dedicated Home Health Aides / Certified Nursing Assistants (CNA) to join our team in Fort Wayne, IN. In this role, you will provide essential in-home care and support to individuals who require assistance with daily living activities, promoting their independence and enhancing their quality of life. Requirements

  • Valid CNA certification or Home Health Aide training.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Reliable transportation to travel to client homes in Fort Wayne, IN.
  • Compassionate, patient, and attentive to the needs of clients.
  • Background check and drug screening may be required.
  • Active HHA/CNA certification (or willingness to complete required HHA training).
  • Strong work ethic and passion for serving the community.
  • Reliable transportation and ability to meet shift expectations.
  • Respectful communication and a commitment to high-quality care.

Responsibilities

  • Assist clients with personal care tasks such as bathing, dressing, grooming, and hygiene.
  • Support clients with mobility and transfers to ensure their safety and comfort.
  • Monitor and report any changes in the client’s condition to the nursing staff or supervisor.
  • Provide companionship and emotional support to clients, fostering a positive and caring environment.
  • Assist with light housekeeping, meal preparation, and medication reminders as needed.
  • Maintain accurate and timely documentation of care provided.
  • Support household tasks such as meal prep and light housekeeping.
  • Provide companionship and help clients remain safe and independent at home.
  • Communicate changes in condition to the office promptly.
  • Deliver care with dignity, patience, and professionalism.
