About The Position

The Home Health Aide (HHA) at Heritage Home Healthcare provides personal care and supportive services to clients in their homes under the direction and supervision of a Field Supervisor and Registered Nurse. The HHA assists clients with activities of daily living (ADLs) and promotes safety, comfort, and independence while maintaining compliance with New Mexico Department of Health (NM DOH) and CMS regulations. Essential Functions: Provide personal care services including bathing, grooming, oral hygiene, dressing, toileting, and incontinence care Assist with mobility, transfers, ambulation, and positioning using proper body mechanics Assist with feeding and meal preparation in accordance with dietary instructions Provide skin care and assist with pressure injury prevention Maintain a clean, safe, and organized client environment related to care tasks Support clients with cognitive impairments, dementia, or behavioral health needs as outlined in the Plan of Care Observe and report changes in client condition, behavior, or environment to the supervising nurse or agency staff Accurately document services provided according to agency policy and timelines Communicate effectively with clients, families, and the care team Follow the Plan of Care and report when services fall outside scope or instructions Follow infection control standards and universal precautions Use personal protective equipment (PPE) appropriately Adhere to patient rights, confidentiality (HIPAA), and professional boundaries Report suspected abuse, neglect, or exploitation in accordance with NM law and agency policy Participate in required training, competency evaluations, and in-service education

Requirements

  • High school diploma or equivalent
  • Completion of a state-approved Home Health Aide training program or documented competency per CMS guidelines
  • Ability to pass background check, abuse registry check, and drug screening
  • Reliable transportation and valid driver’s license (as required)
  • Current CPR/First Aid certification (or ability to obtain)
  • Ability to read, write, and communicate effectively in English
  • Ability to lift, push, pull, and carry up to 50 lbs.
  • Frequent standing, walking, bending, and reaching
  • Ability to assist with transfers and mobility
  • Ability to work in various home environments
