About The Position

Join VNA of Care New England as a Home Health Aide/CNA and provide essential care to patients in their homes. This role offers flexibility, one-on-one patient interaction, and the opportunity to make a meaningful impact. You will work under the supervision of a Case Manager, assisting patients with daily living activities, providing companionship, and monitoring their condition.

Requirements

• High school diploma or equivalent
• Current Rhode Island CNA certification (required)
• CPR certification (required)
• Valid driver's license and automobile liability insurance

Nice To Haves

• At least 1 year of CNA experience preferred
• Home care experience a plus
• A passion for helping others and strong communication skills

Responsibilities

• Assisting patients with bathing, dressing, grooming, and personal hygiene
• Helping with toileting needs and mobility support
• Performing light housekeeping duties, changing linens, and preparing meals
• Reporting food and fluid intake/output as assigned
• Taking and recording vital signs under professional supervision
• Providing medication reminders and emotional support
• Observing and reporting any changes in patient condition or behavior
• Documenting care accurately and communicating with the care team
• Supporting therapy exercises as directed
• Calling to confirm scheduled visits and respecting each patient’s routine
