About The Position

Cardea Health is a non-profit organization dedicated to providing compassionate health care to marginalized populations. Our mission is to create and support programs that protect the health and autonomy of vulnerable individuals and promote equity and social justice to improve the well-being of our entire community. We are currently seeking a Home Health Agency Director for a temporary assignment. This role involves overseeing an agency that includes RNs, LVNs, PTs, and OTs. The Director will be responsible for quality, compliance, strategy, auditing, and overseeing agency personnel. A key aspect of this role is reviewing documentation for quality and compliance, and supporting all agency activities related to client safety, satisfaction, and clinical outcomes. TLC often services clients who reside in unstable and/or non-traditional housing environments, such as shelters or unsheltered areas. Requirements

  • Valid California RN or LVN license
  • CA Driver’s License
  • Current CPR proof
  • Nursing skills according to accepted standards
  • Strong interpersonal and leadership skills
  • Knowledge of home health needs, and compliance requirements
  • Reliable vehicle and valid auto insurance
  • Minimum one year of home health nursing required

Nice To Haves

  • A strong clinical background and an HHA RN with experience in clinical care, administration, quality, and compliance.
  • Proven ability to manage and lead a team, making sound clinical and administrative decisions.
  • Excellent skills for effective collaboration with staff, patients, and families.
  • Ability to identify and resolve issues related to patient care and operations.
  • Deep understanding of federal, state, and local home health laws.
  • Knowledge of budgeting, financial reporting, and resource allocation.

Responsibilities

  • Oversee daily operations, including personnel management, budgeting, and compliance with regulations.
  • Support activities related to licensure, regulations, and reporting.
  • Develop, implement, and maintain policies and procedures related to patient care and agency operations.
  • Participate in budget development and monitoring, ensuring efficient use of resources.
  • Ensure adherence to all federal, state, and local regulations, as well as accreditation standards like those of the Joint Commission.
  • Partner with the HH Nurse manager to orient and evaluate clinical and administrative staff.
  • Contribute to the development of agency goals and objectives and participate in long-term planning.
  • Ensure care is delivered according to established clinical standards and best practices.
  • Apply infection control principles.
  • Monitor patient outcomes, participate in quality improvement initiatives, and ensure high-quality care.
  • Assess agency activities for quality and compliance.
  • Conduct chart audits and supervisory visits.
  • Lead and participate in team meetings, fostering communication among healthcare professionals.
  • Audit patient care plans.
  • Ensure clinical staff are qualified and competent, providing ongoing support and supervision.
  • Stay current with healthcare regulations and ensure agency compliance.
  • Accepting clinical assignments aligned with skills and qualifications.
  • Completing documentation promptly.
  • Participating in continuing education and in-service programs.
  • Communicating effectively with families and staff.
  • Promoting company philosophy and policies.
  • Demonstrating professionalism and ongoing growth.
