Requirements
- Active Minnesota RN license in good standing.
- Strong OASIS and admissions experience.
- Ability to work independently and manage your time effectively.
- Confident communicator with patients, families, and providers.
- Reliable transportation for field-based visits.
Responsibilities
- Complete Start of Care (SOC) admissions and OASIS documentation.
- Perform comprehensive in-home assessments.
- Develop and initiate personalized care plans.
- Coordinate services and communicate with providers and caregivers.
- Ensure safe, smooth transitions into home health services.
