Humber River Health is seeking a Clinical Coordinator, Home Dialysis to join their Nephrology Program. This role involves providing program/service leadership to foster innovation and excellence in patient-focused care. The coordinator will manage a comprehensive approach for patients starting dialysis, liaising with patients, families, nephrologists, and the interdisciplinary team. Key responsibilities include assessments, education, shared decision-making regarding treatment options, and providing support and follow-up to encourage independent dialysis modalities. The position also involves accountability to the Ontario Renal Network (ORN) for independent dialysis assessment and data collection.
