About The Position
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. This role involves establishing and maintaining professional relationships with key physician groups and referral sources, acting as a liaison on behalf of Catholic Home Care. The nurse will identify potential market growth opportunities and program expansion, and serve as a customer service representative, always acting in a professional and supportive manner.
Requirements
- Registered Nurse with current licensure in NYS
- 2 or more years of home care experience preferred
- Customer Service and Marketing skills required
Responsibilities
- Assess, plan, and develop referrals.
- Establish and maintain professional relationships with key physician groups and referral sources.
- Act as Liaison to referral sources on behalf of Catholic Home Care.
- Identify potential market growth opportunities and program expansion.
- Serve as a Customer service representative and act in a professional and supportive manner at all times.
