About The Position
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. This role involves establishing and maintaining professional relationships with key physician groups and referral sources, acting as a liaison on behalf of Catholic Home Care. The nurse will also identify potential market growth opportunities and program expansion, serving as a customer service representative in a professional and supportive manner.
Requirements
- Registered Nurse with current licensure in NYS
- Customer Service and Marketing skills required
Nice To Haves
- 2 or more years of home care experience preferred
Responsibilities
