About The Position
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. This position will travel between Catholic Health's Skilled Nursing Facilities.
Requirements
- Registered Nurse with current licensure in NYS
- 2 or more years of home care experience with patient assessments
- Customer Service and Marketing skills
Responsibilities
- Establish and maintain professional relationships with key physician groups and referral sources.
- Act as Liaison to referral sources on behalf of Catholic Home Care.
- Identify potential market growth opportunities and program expansion.
- Serve as a Customer service representative and act in a professional and supportive manner at all times.
