About The Position

The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on-going case management. This position will participate in weekend/on-call rotation.

Requirements

  • Graduate of an accredited nursing program for LPN or CNA.
  • Graduate of a program approved by the National Board for Certification in Occupational Therapy.
  • Graduate of a Physical Therapy Assistant program approved by the American Physical Therapy Association.
  • 5 years of direct care experience.
  • At least 1 year of experience working with the frail and elderly population.
  • 2 years of supervisory experience.
  • Valid Pennsylvania COTA/L, PTA, CNA or LPN license.
  • Valid PA Driver’s License.
  • Knowledge of the medical, social, and emotional needs of a frail, elderly population.
  • Strong interpersonal relationship skills.
  • Demonstrated ability to effectively collaborate with others.
  • Proactive approach.
  • Working knowledge of durable medical equipment (DME).
  • Crisis intervention skill.
  • Effective written and oral communication skills.
  • Strong organizational and planning skills; ability to manage multiple priorities.
  • Must be able to work independently and to utilize critical decision-making skills.
  • Working knowledge of utilization review, quality assurance and managed health care concepts.
  • Ability to work with the interdisciplinary team approach to care for the elderly.
  • Working knowledge of local health care and geriatric service networks.
  • Basic working knowledge of Windows operating systems, e-mail, word processing.
  • Able to deliver services in a compassionate, responsive, and courteous manner.
  • Able to work effectively with staff, participants, providers, and referral sources.
  • Interest in geriatrics and community-based programming.
