About The Position

Under the direct supervision of the PACE Director of Center Operations, the Home Care Supervisor RN is responsible for coordination, supervision, and delivery of home care services for PACE participants. This role involves supervising staff in their daily activities to ensure the delivery of care, ensuring compliance with regulatory and state guidelines. The supervision extends to Personal Care Assistants (PCA) and Licensed Vocational Nurses (LVN) in their delivery of assistance with participant’s activities in both center and home environments. The position is located in Modesto County and Merced County, with a schedule of Monday – Friday, 8am - 5pm.

Requirements

  • Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
  • Must be able to work effectively in a team environment.
  • Must be able to treat geriatric patients.
  • Only act within the scope of his or her authority to practice.
  • Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel regularly.
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Meet a standardized set of competencies established by Central Valley PACE and approved by CMS before working independently.
  • Current BLS CPR Card certified by American Heart Association or American Red Cross.
  • Minimum two (2) years supervisory or management experience in a health care setting.
  • Minimum two (2) years’ experience working with the frail or elderly population.
  • Current unencumbered CA Registered Nurse (RN) License.
  • Minimum three (3) years of registered nursing experience within the last five (5) years in a home health agency, primary care clinic or health facility.
  • A bachelor’s degree or higher degree in nursing or other health related field.

Responsibilities

  • Managing all home care employees, vendor schedules and finding coverage for vendor or employee call outs.
  • Timely reconciliation of vendor invoices and approving them for payment.
  • Communicating participant changes with team members.
  • Coordinating 24-hour home care services, including nursing, PT, OT, ST, personal care, DME, and contracted services, to ensure participant needs and Plan of Care goals are met.
  • Supervising home care staff and contracted services, including staffing, scheduling, coverage, resource management, performance, evaluations, service quality, and corrective action in collaboration with the Director of Center Operations.
  • Providing after-hours support for clients, referral partners and prospective clients via e-mail, telephone and/or home visits.
  • Assisting in developing policies and procedures as they relate to Home Care Services.
  • Coordinating orientation and onboarding of new home care staff and providing ongoing coaching, education, and operational support.
  • Serving as the PACE Home Care Coordinator, overseeing required assessments, reassessments, care planning, IDT participation, documentation, and home care regulatory requirements.
  • Monitoring home care service delivery and documentation, identifying quality or compliance gaps, and implementing corrective actions as needed.
  • Supporting internal and external regulatory audits, surveys, and monitoring activities related to PACE home care services.
  • Partnering with clinical informatics and operational teams on EMR upgrades, testing, implementation, training, and optimization that impact home care workflows.
  • Evaluating home care processes and recommending workflow and technology improvements to improve efficiency, documentation, compliance, and quality of care.
  • Providing ongoing staff education related to EMR workflows, documentation standards, regulatory requirements, new functionality, and operational process changes.
  • Managing staff performance and employee relations activities, including disciplinary actions, performance reviews, scheduling, payroll reconciliation, budgetary practices, performance improvement activities, and off-hours administrative or clinical support.
  • Participating in IDT, staff, and committee meetings and communicating participant changes to appropriate team members.
  • Observing for changes in participants’ physical, mental, emotional, or social functioning and ensuring appropriate communication and follow-up.
  • Other projects and duties as assigned.
