About The Position

Youngs Healthcare, a Care Advantage Inc company, is seeking a dynamic Home Care Supervisor RN with Bilingual Korean/English skills for their Personal Care Team. This is a PRN opportunity for an experienced nurse with leadership skills. The company specializes in one-on-one quality personal care and companionship in various settings, tailoring care plans to client needs. This position covers the Annadale, VA area and surrounding regions.

Requirements

Must possess an active Virginia Registered Nurse license
Minimum of 1 year in a Nursing role (LPN to RN acceptable)
Minimum of Associates Degree in Nursing
Comfortable working collaboratively in cross-functional teams
Ability to communicate effectively, both verbally and in writing, with all levels of an organization
Excellent presentation skills
Adequate transportation for home visits with mileage reimbursement
Computer/iPad Technology skills for documentation purposes

Nice To Haves

Bilingual Korean/English skills

Responsibilities

Makes initial contact with the patients and explains services offered
Completes start of care assessments and documentation to onboard new patients
Designs an individualized plan of care for each patient
Instructs assigned care team members on the needs and care plan details
Reports progress of care to involved parties
Follows up and maintains relationships with clients, always advocating on their behalf
Establishes relationships with community partners and referral sources
Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes
Reviews caregiver documentation
Takes part in interdisciplinary team meetings to ensure needs of all patients are met
All other duties as assigned by Administrator and job description
