About The Position

Make a Difference as a Home Care RN with Guardian Angel Senior Services! Are you a compassionate Registered Nurse looking for a role where you can make a real difference in the lives of your patients? Join Guardian Angel Senior Services as a Home Care RN and provide meaningful, individualized care to seniors in the comfort of their own homes. This position is based out of Worcester, MA and surrounding towns. You'll have the opportunity to use your nursing expertise while building meaningful relationships with clients, supporting caregivers, and helping ensure the highest quality of care. Home care gives you the opportunity to focus on the whole person, not just a diagnosis. Requirements

Current Massachusetts Registered Nurse (RN) license.
Current CPR certification required.
Strong clinical assessment and documentation skills.
Excellent communication and interpersonal skills.
Compassionate and patient-centered approach to care.
Strong attention to detail.
Ability to work independently and manage multiple client needs.
Comfortable providing education and guidance to caregivers and families.
Reliable transportation and willingness to travel throughout the Worcester service area.

Nice To Haves

Home care experience preferred.

Responsibilities

Perform in-home clinical assessments and evaluate client needs.
Develop and update individualized care plans.
Monitor client progress and revise care plans as needed.
Educate clients and families regarding care plans, treatment, and health maintenance.
Observe caregivers in the field to ensure they are following the client's care plan and providing appropriate care.
Provide caregiver education and re-education when needed.
Communicate with clients, families, caregivers, and other healthcare professionals.
Accurately document visits, assessments, and client progress.
Maintain compliance with applicable state and federal regulations.
Help identify opportunities to improve the quality and safety of client care.
