About The Position

This role involves providing direct patient care as a Home Care RN Case Manager, focusing on assessments, care plan development, and coordination of care. The position requires managing a caseload, administering medications, monitoring patient progress, and educating patients and their caregivers. Collaboration with physicians and clinical teams is essential for ensuring continuity of care. The role also includes supervising Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs), completing accurate documentation (OASIS, visit notes, orders), and ensuring compliance with all relevant policies, regulations, and HIPAA standards.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities
