About The Position
St. Mary’s Home Care and Hospice, located in Athens, GA, is a leading provider of compassionate and exceptional home care services. They offer a comprehensive approach with a focus on quality of care, utilizing a pioneering care model called Home Care Connect, which integrates virtual care for remote monitoring to help patients avoid unnecessary hospitalizations. The Home Care RN position involves providing one-to-one care in patients' homes, utilizing advanced technology and clinical expertise for assessment, care planning, intervention, and supervision of patient care. Nurses must adhere to physician orders and comply with the Nurse Practice Act and organizational policies.
Requirements
- Graduate of an approved nursing education program.
- Licensure as a Registered Nurse in the State of Georgia.
- One (1) year experience as a professional acute care nurse.
- Home care experience (preferred but not required).
- Oasis experience (required).

Responsibilities
- Provide one-to-one care with patients in their homes.
- Perform client assessment, care planning, and intervention.
- Supervise overall patient care and outcomes.
- Follow physician orders.
