The Home Care Physical Therapy Case Manager is responsible for conducting physical therapy evaluations, establishing plans of care, and implementing treatment goals for patients in their homes. This role involves interpreting physician referrals, performing comprehensive patient assessments, and continuously evaluating patient response to treatment. The Case Manager will update and revise care plans as needed, provide regular status reports to physicians, and effectively communicate any patient issues or changes to both physicians and management. A key aspect of the role is maintaining accurate clinical and patient records in compliance with state and federal guidelines, managing an assigned caseload for optimal functional and financial outcomes, and acting as a patient advocate to ensure the best possible results. The position may also involve supervising Physical Therapist Assistants (PTAs), conducting joint visits with Home Health Aides, and teaching appropriate treatment procedures and adaptive device plans to patients and caregivers. Additionally, the Case Manager will participate in chart audits, quality improvement activities, and facilitate discharge planning by utilizing community resources for continuity of care. This role also serves as a resource for other employees, including training, orienting, and mentoring, and may act as an instructor for students in the Clinical Student Program. The role requires performing patient positioning and transporting duties, which involve lifting and pushing/pulling with proper technique, and demonstrating age-specific knowledge and skills for patient care.
