About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in and around Westbury. The role involves evaluating patients in their homes, developing individualized plans of care, delivering treatment, and coordinating with patients, families, physicians, and interdisciplinary team members. All experience levels are encouraged to apply, and the organization will train the right new graduate. Home health experience is preferred but not required; a mentor is available to support documentation and compliance. The position offers flexible scheduling and autonomy, with the option to see patients any seven days of the week. The role is available with 1099 and W-2 work-style options.

Requirements

• Graduate of a Physical Therapist program.
• Licensed Physical Therapist in New York or eligible for licensure.
• Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Accept and treat patients within the designated geographic area.
• Evaluate patients’ physical therapy needs in the home setting and develop individualized treatment plans.
• Develop plans of care and treatment goals in consultation with physicians, patients, and families.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, particularly regarding discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
