Requirements

Licensed Physical Therapist (PT) in Wisconsin
Passion for providing home care services
Strong clinical reasoning and treatment planning skills
Excellent communication and collaboration abilities
Energy, initiative, and a desire to grow professionally
Commitment to integrity, compassion, and excellence
Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
One year practicing as a Physical Therapist

Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans including OASIS paperwork.
Deliver high-quality, evidence-based physical therapy services.
Supervise PTAs as appropriate.
Collaborate with an interdisciplinary team focused on exceptional outcomes.
Build strong relationships.
Contribute to a positive, team-driven therapy culture.
Maintain accurate and compliant documentation.
