About The Position

Prime Healthcare Home Health is seeking a Per Diem Occupational Therapist to join their team. This role involves performing evaluations and re-evaluations for patients to establish care plans and goals. The therapist will conduct patient treatments using various occupational therapy procedures, including modalities, therapeutic exercise, and ADL training. Communication with physicians, staff, patients, and caregivers is essential, as is supervising Home Health Aides' care related to the patient's rehab program. Active participation in home care department functions is also expected.

Requirements

Bachelor’s degree or Master’s degree in Occupational Therapy.
Current license as an Occupational Therapist.
One (1) year recent clinical experience.
Current BLS certification.
Current driver’s license, car insurance, car registration, and reliable automobile.

Nice To Haves

Home Care experience preferred.

Responsibilities

Performs evaluations and re-evaluations on patients to establish plan of care and goals.
Performs patient treatment per Occupational Therapy procedures of modalities, therapeutic exercise, ADL training and other accepted methods.
Communicates pertinent information, verbally or written, to physician, other staff members and patients/caregivers.
Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.
Demonstrates active participation in home care department functions.
