Home Care Nurse - RN - LPN
About The Position
BrightStar Care is seeking a dedicated Home Care Nurse (RN or LPN) to provide compassionate care to a 19-year-old male client with a Traumatic Brain Injury (TBI). The client is wheelchair-bound, has limited speech, is hemiplegic, and requires a nurse with neurological and/or behavioral health experience. This role involves medication management, in-home assessments, monitoring client condition changes, and collaborating with the client's family. Requirements
- At least 1 year nursing experience with neurological and/or behavioral health
- At least 1 year nursing experience in home
- Current unrestricted RN or LPN license in Utah
- Certified CPR Certification
- Negative TB skin test or chest x-ray
- Must be able to pass a drug test and a background check
- Licensed driver with valid automobile insurance
- Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
- Must be able to lift 75 pounds

Responsibilities
- Medication set up and oversight
- Provide thoughtful care to clients based on personalized Plan of Care
- Provide in home assessments
- Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
- Collaborate with client's family on plan of care, health changes, and concerns
