Requirements
- Active Registered Nurse (RN) or Practical Nurse (LPN) license required for the state of Minnesota
- Must pass a Minnesota background study
- Compassionate, patient-focused approach to care
Responsibilities
- Perform lab draws and medication management support
- Set up and assist with medication organization
- Serve as a liaison between clients and physicians
- Complete patient assessments and accurate documentation
- Supervise Personal Care Assistants (PCAs)
- Support coordinated, patient-centered care plans
- Ensure continuity of care and communication across providers
