Requirements

• Nurse Practitioner or Physician Assistant with 1 + years of experience in Home Health, or a related field.
• TB screening test required.

• Bilingual in Spanish and English

Responsibilities

• Conduct comprehensive in-home evaluations and examinations
• Use both in-person and telemedicine capabilities to deliver care
• Develop and implement personalized treatment plans
• Manage acute clinical issues in the home
• Maintain accurate documentation
• Participate in medical team meetings
