About The Position

Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Home Care Manager to oversee our home-care services in the Fortuna area. The schedule for the position is Monday through Friday, 8 am to 5 pm. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. The pay for the position will be based on licensure (LVN or RN), years of experience, and education. JOB SUMMARY: Under the supervision of the Director of Redwood Coast PACE, the Home Care Manager is responsible for the development and implementation of home care services for program participants, including the coordination of durable medical equipment. The Home Care Manager directly supervises the in-home CarePartner team.

Requirements

  • Bachelor of Science degree in Nursing preferred.
  • At least two (2) years of experience as a RN/LVN required, preferably in home or community health care.
  • One (1) year working with a medically frail or elderly population required.
  • One (1) year of supervisory experience required.
  • Current Registered Nurse licensed by the California Board of Registered Nursing, or BVNPT Vocational Nurse Licensure required.
  • Current CPR and First Aid certification within six (6) months of hire.
  • Valid California Driver’s License, and personal auto liability insurance required.
  • Must have a personal vehicle in good driving/operating condition and insured for State minimum liability requirements that can be used for the applicable job functions noted above.
  • Requires clearance of a DOJ and FBI criminal history background check
  • Pre-employment physical and PPD skin test.
  • Requires Motor Vehicle Report verification.
  • Ability and interest to work with the frail, elderly, and disabled adults to enhance and encourage their independence.
  • Nursing knowledge and skills necessary to navigate complex clinical situations.
  • Knowledge of physical, social, and mental health programs operating within a licensed health care facility or clinic.
  • Strong administrative, organizational, and supervisory knowledge, skills, and abilities.
  • Ability to coordinate programs and services.

Responsibilities

  • Provides day-to-day oversight of home care services for PACE participants.
  • Supervises the Home Care Assistant, Home Care Schedulers, and Home Care Supervisors in coordination of home care services.
  • Follows HSRC hiring practices including providing orientation, education, supervision and corrective action for the in-home CarePartner team.
  • Oversees and monitors the home care provided by in-home care contract agencies.
  • Acts as a member of the Redwood Coast PACE Management team.
  • Using the Home Care Safety Assessment process, oversees and/or assesses the home care needs of frail elderly participants.
  • Ensures all assessments are completed and documented in a timely manner.
  • Conducts home safety visits as assigned to ensure completeness of the PACE enrollment process.
  • Works closely with home care agencies to orient, educate, supervise and evaluate, in-home CarePartners.
  • Works closely with the Quality Assurance Coordinator to oversee all activities related to quality assurance of the home care program.
  • Works closely with the Center Manager and Quality Assurance Coordinator to receive, document and resolve reports of participant rights violations, grievances, appeals and other issues related to service and quality of care.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Director of Redwood Coast PACE.
  • Takes necessary measures to ensure a safe environment for co-workers, contractors, participants, visitors and others.
  • Reports appropriately any signs and conditions of abuse.
  • Consistently meets or exceeds Redwood Coast PACE targets for productivity.
  • Continually seeks better ways for delivering services and communication with participants.
  • Consistently meets or exceeds Redwood Coast PACE customer service, quality assessment and performance improvement targets.
  • Demonstrates respect for and promotes participant rights including dignity, self-determination, access to care, confidentiality and independence.
