About The Position

Our Phoenix Skilled Office is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Skilled Nursing team in either pediatric and geriatric private duty roles. We believe our clients and their families deserve the highest quality care- delivered with compassion, excellence, and reliability. This role involves providing one-on-one home care, offering traditional shifts (4’s, 8’s, 10’s or 12’s) with hands-on care for only one client per day. You will build client relationships and witness their milestones. Your day will involve traveling to a client's home, following doctors' orders, performing assessments, monitoring conditions, documenting observations, and performing skills with autonomy.

Requirements

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• A current nursing license in good standing in the state of Arizona
• One year of experience working as a Licensed Practical Nurse

Nice To Haves

• Trach experience preferred, but not required

Responsibilities

• Travel to one client’s home near you on a schedule you choose
• Follow up with, execute, and properly document doctors' orders
• Perform assessments and monitor clients' conditions
