About The Position
MAS Home Care is seeking dedicated Licensed Practical Nurses (LPNs) to provide personalized care to patients, primarily in their homes. This role addresses a critical need in healthcare and offers a flexible option for earning supplemental income. The position involves collaborating with caregiving teams, patients, and families to ensure the highest quality of care, promote client independence, and maintain safety. This role requires a patient, trusting, communicative, and empathetic individual.
Requirements
- Must have an active LPN license in the state of New Hampshire
- 1 year prior experience
- Active CPR certification
- Valid driver’s license and reliable transportation
- Clean driving record and background check
Responsibilities
- Collaborate with the caregiving team, patients, and families to provide the highest level of care.
- Ensure the health and safety of clients while promoting independence.
- Evaluate clients and develop care plans for clinical and non-clinical teams to follow.
- Provide training and education to patients, families, and care teams to ensure safety in the home and promote independence.
- Work alongside the care team to develop care plans and complete documentation.
- Assist with promoting client independence and healing.
- Demonstrate competency in carrying out patient care/treatments.
