Position Summary: The Home Care Services Liaison is responsible for building and maintaining relationships with community referral sources. This role serves as a key link between the agency and the community, ensuring awareness of services, supporting growth initiatives, and ensuring smooth transitions for patients onto home care and hospice services.

Requirements

  • 1-3 years of experience in healthcare marketing, home care, hospice or related field.
  • Strong knowledge of home care and hospice services and referral processes
  • Excellent communication, customer services, and interpersonal skills
  • Self-motivated, goal-oriented, and able to work independently
  • High school diploma
  • Relationship building and networking
  • Sales and business development
  • Communication and presentation skills
  • Organization and time management
  • Problem-solving and critical thinking
  • Primarily field-based with travel within service area
  • Occasional office work for meetings and reporting
  • Flexible schedule may be required to attend community events

Responsibilities

  • Build and maintain strong relationships with hospitals, medical practices, SNFs, ALFs and other referral sources
  • Educate referral sources on available home care and hospice services and admission criteria
  • Generate referrals through ongoing outreach, networking, and relationship management
  • Conduct regular in-person visits and follow-ups with referral sources
  • Collaborate with intake and home care/hospice leadership to ensure smooth patient transitions and admission to services
  • Represent the agency at community events, health fairs, and meetings
  • Stay informed on industry trends, regulatory changes, and competitor activity
