About The Position
Coordinates and directs homecare services with physicians, nurses and hospital discharge planning staff to decide patient’s appropriateness for homecare.
Requirements
- Graduate of an accredited RN program.
- Previous experience in a certified home health agency.
- License and current registration to practice as a registered Professional nurse in New York State.
