About The Position
Responsible for the overall coordination of the referral and admissions process. Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care. Hours: Sunday-Thursday 11:30am-8:00pm PST
Requirements
- 1 year of Home Care Sales Required
- Be eighteen (18) years of age or older
- Ability to complete necessary documentation appropriately, legibly and timely
- Ability to handle stressful and delicate situations
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
- Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
Nice To Haves
- Knowledgeable in all Home Health and/or Hospice processes
- At least one (1) year experience in a homecare, hospice and/or healthcare related field preferred
Responsibilities
- Provide explanation of the benefits to patients, families, and community contacts
- Assist with finding alternative services when patient is not a candidate for hospice
- Oversee scheduling of initial nursing assessment and admission
- Maintain admission packets and ensure that proper forms are available to admissions staff
- Assist community outreach representatives with marketing activities to promote relations with referral sources
- Assist with completion of admission paperwork as needed
- Adhere to work schedule and maintain good punctuality and attendance record
- Provide telephone and clerical support to other staff members as time allows
- Maintains accurate clinical records, keeping track of goals and progress of clients
- Abides by all regulations, policies, procedures and standards
- Performs other duties as assigned
- Maintains positive internal and external customer service relationships
- Maintains open lines of communication
- Plans and organizes work effectively and ensures its completion
- Meets all productivity requirements
- Demonstrates team behavior and promotes a team-oriented environment
- Actively participates in continuous Quality Improvement, educational programs, and staff meetings
