About The Position

Seeking a Homecare Infusion RN for Middlesex/Monmouth/Ocean counties. Hackensack Meridian Health is looking for nurses who feel overwhelmed and spread too thin in their current jobs, offering a return to home care where nurses can fall in love with nursing again. The role focuses on one patient at a time, allowing for valuable and meaningful 1:1 time with patients in their homes. This position offers a flexible schedule in a setting with fewer distractions. Requirements

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • Minimum of one (1) or more years of RN Experience.
  • Ability/Experience with Inserting IVs.
  • Competency Requirements: With previous and recent IV experience (preferred) within one (1) year: Demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines.
  • Or Without previous experience or experience not within one (1) year: Will complete a vascular access device (VAD) course through HMH and follow up practicum experience in HMH outpatient infusion setting and return demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines.
  • Successful Completion of Infusion Test within probationary period with grade of 80% or higher.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.
  • NJ State Professional Registered Nurse License.
  • BLS AHA Basic Health Care Life Support HCP Certification.

Nice To Haves

  • Bachelor of Science in Nursing Degree (BSN).
  • Previous and recent IV experience within one (1) year.

Responsibilities

  • Providing professional nursing care to patients in their home through assessment, development, implementation and evaluation of home nursing care needs on an individual and continuous basis of assigned patients in accordance with organization policies and procedures and external regulations.
  • Demonstrating competency with infusion therapy.
  • Assuring compliance with appropriate physician orders in a timely manner and that visit frequency follows physician orders.
  • Assuring documentation reflects compliance with physician's orders and addresses progression to goals being met at discharge.
  • Assuring that assessment, planning, intervention and evaluation of patient care follow policy, procedure, and standard of care.
  • Demonstrating ability to care for a patient and significant other in a holistic manner.
  • Assuring safe patient care is provided.
  • Demonstrating knowledge and competency in completion of assigned treatments and procedures according to established standards and policies.
  • Utilizing an interdisciplinary team and accurately documenting all activities.
  • Assuring clinical judgments and critical thinking reflect professional standards of care and codes of ethics which provide for safe and effective patient outcomes.
  • Appropriately documenting clinical decisions through the appropriate EMR system.
  • Identifying appropriate payors and remaining in compliance with authorizations for self and other disciplines as necessary.
  • Meeting payor source guidelines.
  • Maintaining visit frequency with authorization range.
  • Utilizing secure messaging to insurance nurses for all authorizations.
  • Assuring documentation of assessment, interventions and planning meet organization time lines.
  • Completing nursing assessment, medication reconciliation and collaborating with physicians on Plan of Care.
  • Updating care plan within two week period of change in patient status.
  • Implementing specific teaching strategies to meet established goals on the plan of care.
  • Assuring documentation reflects appropriate teaching strategies in the medical chart that are consistent with goals and plans.
  • Reporting patient care problems to manager or appropriate nursing management personnel and/or physician in a timely manner with possible solutions.
  • Assuring documentation indicates case communication.
  • Appropriately utilizing a manager as a resource.
  • Acting as liaison among physicians, Manager of Clinical Practice and other members of the health care team to ensure effective coordination and appropriate resource referrals.
  • Assuring that coordination of services is effective and meets client's needs.
  • Communicating pertinent patient information and Plan of Care during transfers of patient care loads, either on a daily basis or whenever vacation days/time-off are scheduled.
  • Completing routine assignments accurately, consistently and independently in a timely manner without the need for supervision.
  • Meeting productivity requirements on a consistent basis.
  • Maintaining awareness of new/updated standards and incorporating these into nursing practice throughout the continuum of care.
  • Assuring documentation in the medical chart demonstrates knowledge of current practices' guidelines.
  • Performing other duties and/or projects as assigned.
  • Demonstrating flexibility with job responsibilities in all areas.
  • Adhering to HMH's Organizational competencies and standards of behavior.
  • Lifting a minimum of 50 lbs., pushing and pulling a minimum of 50 lbs. and standing a minimum of 8 hours a day.
