About The Position

The Home Care Coordinator helps ensure clients receive high-quality, consistent care and exceptional service. Serving as a key liaison between clients, families, caregivers, and the Home Care team, this position coordinates services, supports client onboarding, conducts supervisory visits, monitors satisfaction, and helps maintain individualized care plans.

Requirements

  • Associate degree or equivalent experience in healthcare, senior services, social services, or a related field.
  • At least one year of experience in home care, healthcare, senior living, case management, client services, or a related field.
  • Experience working with older adults; experience with individuals living with dementia is preferred.
  • Valid Illinois driver’s license with a good driving record.
  • Strong communication, problem-solving, and decision-making skills.
  • Ability to work independently while collaborating effectively with a team.
  • Strong organizational skills and the ability to manage multiple priorities.
  • Computer proficiency.
  • CPR and Food Handler certifications required within 30 days of hire.
  • Commitment to regular and reliable attendance.

Nice To Haves

  • Experience with individuals living with dementia is preferred.

Responsibilities

  • Coordinate the onboarding of new clients, including welcome calls, service orientations, and communication with clients and families.
  • Assist with client assessments and the development, implementation, and review of individualized care plans.
  • Schedule and conduct supervisory visits to monitor service quality and client needs.
  • Monitor client satisfaction and address concerns to support a positive experience.
  • Provide compassionate support and guidance while treating every client with dignity and respect.
  • Communicate effectively with clients, families, caregivers, and team members to promote continuity of care.
  • Support the Home Care Manager with daily operations and client needs.
  • Participate in the department’s on-call rotation as assigned.
