About The Position

We are seeking a Home Care Coordinator (LPN) who is passionate about senior care, enjoys collaborating with interdisciplinary teams, and thrives in a role where organization, clinical expertise, and genuine compassion come together to make a lasting impact. As the Home Care Coordinator (LPN), you will play a vital role in coordinating and overseeing in-home services for Elderhaus participants. Working closely with our interdisciplinary team, caregivers, participants, and families, you will help ensure that each participant receives personalized, high-quality care designed to support their health, safety, and independence. This position is ideal for an LPN who enjoys relationship-building, problem-solving, and serving as a trusted advocate for older adults.

Requirements

Current, unrestricted North Carolina Licensed Practical Nurse (LPN) license.
Strong assessment, organizational, and critical-thinking skills.
Excellent interpersonal and communication abilities.
Compassionate, participant-centered approach to care.
Ability to manage multiple priorities in a collaborative healthcare environment.
Proficiency with electronic health records and basic computer applications.
Valid driver's license and reliable transportation.

Nice To Haves

Experience in home health, senior care, long-term care, PACE, case management, or care coordination preferred.

Responsibilities

Coordinate and monitor home care services for PACE participants.
Conduct home visits and ongoing participant assessments.
Collaborate with the interdisciplinary team to develop and implement individualized care plans.
Communicate regularly with participants, families, caregivers, and healthcare providers.
Monitor participant status and identify changes in condition that may require intervention.
Coordinate caregiver schedules and services to ensure participant needs are met.
Provide education, guidance, and support to participants and families.
Maintain accurate clinical documentation and regulatory compliance.
Participate in care conferences and team meetings focused on achieving positive participant outcomes.
Promote participant safety, independence, and quality of life in the home setting.
