About The Position

In this role, you'll care for patients in the Fort Wayne Area as a member of a Home Care team. You will be working 40 hours each week, Monday through Friday, with weekend rotations. Daytime Hours. Stillwater Hospice has been caring for the seriously ill of Northeast Indiana since 1888, and our goal is to foster the optimal quality of life through medical, spiritual, and emotional support for patients and their loved ones. We provide high-quality, patient-focused, and compassionate care that helps terminally ill patients and their families live as fully as possible. Providing hospice care allows patients and their families to have meaningful moments together.

Requirements

High school diploma or GED required.
Completion of 105-hour CNA Certification program (need copy of Certificate).
Current/active CNA License
Minimum six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months, in the home health, long-term care, or hospital setting.
RN nursing students who have successfully completed courses with a grade of “C” or better, who currently possess an active CNA License, will be said to have met the above qualifications.
Must hold current CPR certification or obtain within the orientation period.
Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her or others in the workplace.
Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
For CNA Home Care, must be able to drive independently to all patient/family/ clients’ homes.
Must have visual, hearing, and mental ability to initiate and comprehend written and verbal communication.
Must be able to engage in clear, concise verbal/non-verbal communication.
Must be able to climb a minimum of three (3) flights of stairs.
Recognizes emergencies and has knowledge of emergency procedures according to agency policy.
Must have the ability to remain calm in occasional emergency situations.
Must be willing to travel in inclement weather.
Must be willing to accept assignments without regard to disease or ethnic origin.
Must exhibit dependability in areas such as attendance, tardiness, and flexibility for unit coverage.
Must be willing to work scheduled holiday rotation.

Responsibilities

Gives client care per agency policies and procedures, following written Plan of Care.
Appropriate and safe techniques in personal hygiene and grooming that include: bed bath, sponge bath, tub bath or shower bath. Shampoo: sink, tub, or bed. Nail and skin care. Oral hygiene. Toileting and elimination needs, emptying commode and catheter bag; empty, cleanse, and reinforce ostomy bag. Normal range of motion and positioning, ambulating with use of gait belt. Changing linens, adequate nutrition, fluid intake and output.
Assists patients with various rehabilitation activities under the direction of a licensed therapist or RN.
Takes and records temperature, pulse, respirations, and blood pressure when assigned.
Meal preparations.
Assists patient with feeding and feeds patients unable to feed themselves.
Observes patient’s condition and reports significant changes in status to the appropriate nurse.
Computer documents care per agency policies and procedures.
Manages care of assigned patients in a timely fashion.
Maintains clean, safe, and healthy environment in the patient care area.
As a member of the interdisciplinary team, participates in patient care conference as assigned to promote holistic care.
CNA will maintain good personal hygiene and conduct themselves in a polite and professional manner.
CNA must maintain a therapeutic relationship with patients. This includes confidentiality of personal matters as well as patients’, coworkers’, and Agency business. Maintains respect for the patient's privacy and property.
Attends educational opportunities and in-services in accordance with agency guidelines to improve clinical practice.
Must be willing to be cross-trained in other departments within the agency.
Any other duties assigned by supervisor.
Adheres to all Stillwater Hospice policies.
Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
Participates in quality improvement activities and/or other agency committees or task forces when requested.
Performs light housekeeping chores.
May run errands, on occasion, with supervisor approval.
