About The Position

Live Long Well Care is seeking Full Time and Part Time CNA/Caregiver/Home Health Aides/Companions to provide personal care and companionship for seniors. Day and night positions are available, as well as weekend work. CNA certification is not required but is a plus and paid a premium. A valid Driver's License and clean driving record are necessary. Services will be provided to various clients within a 30-mile radius of the branch. An up-to-date smartphone is required for electronic viewing and capture of Time and Attendance, Care Tasks, and Communication. Requirements

  • Must have a valid Driver's License and clean driving record.
  • Must have an up-to-date smartphone for the electronic viewing and capture of Time and Attendance, Care Tasks, and Communication.
  • High school graduation or GED required.
  • Ability to read, write, speak, and understand English fluently.
  • Ability to work with minimal or close supervision.
  • Must conduct all business with a professional manner and with a high level of confidentiality.
  • Must pass competency testing on hire and ongoing for meal preparation, housekeeping, infection control, safety in the home, proper handling of emergencies, transportation, dog walking and care.
  • Possess the physical ability to perform job-related duties which may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling and the ability to provide partial or complete assistance with non-personal activities of daily living, without assistance from another healthcare worker, or significant other.
  • Ability to work in time-sensitive or stressful situations.
  • Sit in a chair/stand for extended periods.
  • Ability to commute to various client locations throughout the work day/week.
  • Eligibility to drive on company business and a satisfactory driving record required.

Nice To Haves

  • CNA certification is not required, but can be a plus and is paid a premium!
  • Two years’ experience in a similar position desired.
  • Two years’ experience working with the elderly desired.
  • Current listing with no substantial finding on state Aide Registry, as state appropriate, if applicable.

Responsibilities

  • Provide general assistance and care to patient/clients in accordance with the non-personal care plan.
  • Plan meals, purchase food and household supplies, and prepare and serve nutritious meals.
  • Provide for a clean, safe, and orderly environment for the patient/client and family.
  • Perform housekeeping activities such as washing dishes, cleaning the kitchen, and laundering clothes and bed linens.
  • Provide companionship for the patient/client as required.
  • Provide medication reminders.
  • Perform other duties, such as transportation of the independent patient/client, when directed by the supervisor.
  • Accurately and thoroughly document services provided in accordance with agency policy.
  • Immediately report any changes in patient/client’s needs, condition, or incidents to the supervisor.
  • Establish and maintain effective communication and a professional relationship with patient/clients/families and co-workers.
  • Participate in case conferences as appropriate.
  • Maintain and enhance skills through attending appropriate staff development programs.
  • Fulfill assignments with reliability and punctuality.
  • Maintain a cooperative manner towards patient/client/family members and the healthcare team.
  • Listen and respond to patient/client’s needs in a sensitive and timely manner.
  • Assist the supervisor by providing feedback regarding the non-personal care plan.
  • Participate in Quality Assessment and Improvement activities.
  • Attend, participate in, and/or conduct internal staff development programs, and obtain continuing education.
  • Maintain and enhance clinical practice skills.
  • Perform other duties as assigned.
  • Maintain strict confidentiality of patient and employee information.
  • Maintain strict confidentiality of Agency proprietary and business information.
  • Report for work on time and prepared to assume duties.
  • Adhere to company dress code.
  • Record work accurately and legibly.
  • Follow policy for reporting sick or absent.
  • Schedule all time off with supervisory approval.
  • Vary work schedules to meet Agency needs.
  • Adhere to company policy, procedure, and processes.
  • Respond to internal and external customers in a professional and friendly manner.
  • Offer assistance as needed or direct individual to appropriate resources with proper follow through.
  • Wear ID badge in a visible location, if required.
  • Comply timely with all requirements related to risk management, safety, infection control, TB screening, security, and fire.
  • Provide a safe environment for clients, employees, and others.
  • Assume responsibility for all mandatory in-services, certification, and/or licensure, competencies, etc.
