Immediate Need Experienced Caregiver (Home Care Aide)
About The Position

Requirements
- Prior 3+ year experience in home health care, assisted living facilities, nursing homes, or long-term care environments
- HCA highly desired
- Up-to-date car insurance in your name, registration and valid Driver's License
- Experience working with patients requiring hospice or dementia or memory care services is advantageous.
- Understanding of/Willingness to learn medical terminology relevant to patient care scenarios.
- Proven ability to handle sensitive situations with compassion while maintaining professionalism
- Reliability on meeting commitments and open communicator

Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
- Maintaining a clean, safe, and healthy environment within the client's residence.
- The performance of all personal care activities contained in a written Plan of Care
- Monitor patient health by observing and recognizing changes in condition, and reporting findings accurately using Assisting Hands system.
- Support clients that could include specialized needs including Alzheimer’s care, dementia care, hospice care, etc.
- Engage in home & community care activities that support clients’ independence while respecting their privacy and preferences.
- Abide by policies and procedures of Assisting Hands Santa Clara and CDSS and HIPAA guidelines
